Qualifications Education Diploma of Practical Nursing is required Experience 1-2 years of experience as an LPN required in a clinical setting or case management role Certifications, Licenses and Registrations Licensed LPN in the state of practice is required CPR certification is required Certification as a breathe DOT substance testing collector within 30 days of employment is required Certification as an urine DOT substance testing collector within 30 days of employment is required.
